HOPE and SHARE Programs

Resource type: Grants

Granted to Beth Israel Medical Center

Goal

To provide partial support for the HOPE and SHARE programs which help to maintain the elderly’s health and independence following their hospitalization at the Medical Center. HOPE (Homebound Outreach Project for the Elderly) was developed with the dual objectives of enhancing the quality of life of isolated elderly patients by providing opportunities for socialization, and easing the transition from a hospital stay to a return home by maintaining a link with BIMC volunteers who provide companionship. SHARE (Subsidized Homecare for the At-Risk Elderly) helps to ease the transition from a hospital stay to living at home by providing, for a limited time, free home care services.
